You may download these forms for your use as needed. The Word documents are in rich text format and may be opened in Word or WordPerfect.  The format of these files will change, based upon your printer type, but you may type directly into the document.  The Acrobat Reader format (.PDF) files will maintain their format, but you cannot type directly into them.

Applications (to Programs, to Graduate)
Applicants may print the form to complete it and mail or bring it to the Graduate School Office. It is recommended that applicants use the online application which is easy to use and quicker to complete. Apply online at http://www.madonna.edu/pages/gradstudapplyonline.cfm
		Application for certificate (Use this form to apply for a graduate certificate. MBA students must apply for certificate separately from degree.
		Application for degree (Use this form to apply to graduate. Once you have completed the form, take or send it to Student Accounts Office to pay your $50 fee (additional $10 if you miss the deadline found in the Schedule of Classes or posted in the Graduate School Office and the Registrar's Office). After you pay your graduation fee, submit the form to the Graduate School Office. This will initiate an audit and you will receive a letter informing you of the courses you need to complete in order to graduate.)
		Application to Graduate School (Please use this application for applying to one of our Graduate Programs if you prefer to send a hard copy. It is recommended that you apply directly online for faster handling of your application.)
Change of Program within Department
		Change of Program within Department (This form is used to change your specialty within a department and may not be used to apply to a different department or college.)
Criminal History Statement Form
		Criminal History Statement Form (This form is one of the application documents for Master of Science in Clinical Psychology applicants. Complete the form and enclose it with your Personal Statement and any other materials not submitted electronically.)
Independent Study Form
		Independent Study Form (Complete this form with your instructor. Fill in all blanks. Specify the plan for completing the study. Indicate if it will follow the syllabus of a standard course.)
MIGS Form (Graduate Guest Pass)
		MIGS Form (The Michigan Intercollegiate Graduate Studies form is used with permission of the student's advisor and Dean of the Graduate School to take a class as a guest graduate student at another Michigan institution.)
Recommendation Form
		Recommendation Form (This form may be used for the recommendation from a professional associate or supervisor. Provide the form to your reference and ask them to send it either in a letterhead envelope from their business or an stamped envelope that you provide. They should sign over the seal to indicate it was not viewed by you.)
Statement of Purpose Form
		Statement of Purpose Form (This form is to be used by applicants to the MALS, MBA, and nursing programs.)
